The National Transportation Safety Board said Wednesday that very little rubber was left on the jet's wheels and the brakes were severely damaged when the plane crashed on takeoff just before midnight Sept. 19.
The plane's cockpit voice recorder has indicated that crew members thought a tire blew as they hurtled down the runway before failing to abort the takeoff.
Such a simple issue -- why were the tires not checked before takeoff?
